The Department of Homeland Security requires that this form be completed to verify your identity. Read the instructions and only complete Section 1, page 7 and sign. Provide the address where you currently live (no PO Box).
Foreign nationals should use their entry date.
Please bring appropriate documentation (no copies) to your check-in appointment with Lea Fraser. All documents must be unexpired. A list of acceptable documents is listed on page 9. You can either bring one item from List A or one item from List B and one item from List C.

This is the Massachusetts Employees Withholding Exemption Certificate. Complete and sign.
Forms for Foreign Nationals OnlyForeign National Information Form (FNIF) (pdf)
This form must be completed before you can receive any form of payment. All applicable questions on both pages. A copy of both sides of your I-94 Form "Arrival and Departure Record", (a small white card inside your passport), copy of your U.S. VISA from your passport, and 1-20 or IAP66 must be attached to this form. This form must be returned before any check can be issued by the WHOI Payroll Office and must also be completed by anyone receiving tuition remission/scholarship. If you do not yet have a Social Security number please attach receipt of the application for the social security number. Social Security Office in Falmouth
It is essential that you begin the process of obtaining a US social security number right away and report your number to the Academic Programs Office as soon as possible.
